Pinnacle Health Management (PHM) is seeking part time licensed psychologists, clinical nurse specialists/nurse practitioners and licensed clinical social workers to join our team of clinicians providing mental health support to elders in acute inpatient, assisted living and long term care facilities in Southeastern Massachusetts. PHM clinicians partner with the nursing home’s multidisciplinary team of health care providers that include NP’s, PA’s, RN’s, physicians, psychiatrists, psychologists, social workers and activity therapists in meeting the health care needs of elders. By integrating psychiatric and medical treatment, our program provides a comprehensive approach to patient care. Opportunities exist for:

Licensed Clinical Psychologist (Ed.D., Psy.D., Ph.D., D.Min.)
Licensed Independent Clinical Social Worker (LICSW)
Clinical Nurse Specialist (RN/CS)
Nurse Practitioner (NP)

The Psychologist and LICSW specialize in conducting diagnostic assessments, developing behavioral treatment plans and providing one-to-one therapy. The psychologist will also be called upon to evaluate a resident to aid the primary care physician in determining a resident’s capacity to make informed decisions, completing medical certificates to guide the Court’s decision on competency, etc…

The Clinical Nurse Specialist / Nurse Practitioner coordinates PHM service delivery in each home served. Working closely with the residence staff, the CNS/NP will initiate service delivery upon request of the resident’s primary care physician. S/he will complete a diagnostic assessment and provide the PCP with recommendations for psychiatric medication management, therapy, behavioral care planning, and related services. The CNS/NP role is one of practitioner, consultant, educator, clinical expert, and mentor/coach.

General duties subscribed to all practitioners will include: monitor patient progress; maintain patient records; consult with supervising physician when patient’s progress does not meet anticipated and/or predetermined criteria; refers for consultations as appropriate.

Qualified applicants will have an advanced degree from an accredited university and be licensed by their respective Board to practice at the independent level and without restriction. In addition, Clinical Nurse Specialists and Nurse Practitioners will be ANCC certified and have active DEA and CSR certificates.
